What information is required when making an appointment?
When making an appointment you will need name and date of birth or member's DoD ID number.
In order to ensure effective communication between patients and Primary Care Managers our staff will ask you to verify your home phone, work phone, home address, and any allergies to medications to keep your information current.

Outpatient Services schedules over 3,500 appointments each month.
Enrollment in the Defense Enrollment Eligibility Reporting System (DEERS) is mandatory for appointment scheduling. TRICARE Prime patients have priority over TRICARE Select patients.
If you no longer need your appointment, it is recommended to cancel your appointment at least two hours in advance. This allows other patients an opportunity to access health care.
