Occupational Health Clinic
The Occupational Health Clinic is located on the 1
st deck of the hospital within the Preventive Medicine and Audiology Departments. We provide employee-centered occupational health services to prevent illness and disease, fostering readiness for active duty and civilian personnel.

Medical Surveillance Examinations
Medical Surveillance Examinations: Medical surveillance programs are designed to protect workers who are exposed or potentially exposed to hazardous substances in the workplace. Placement of personnel in medical surveillance programs is based primarily on the results of the Industrial Hygiene Survey completed by the Industrial Hygiene Department. Please bring your
SECNAV Form 5100/1, “Supervisor’s Medical Surveillance and Certification Exam Referral”, to your part 1 exam. The top portion of this form is completed by your supervisor. SECNAV Form 5100/1 informs the occupational health team which exam is required and is the method in which the occupational health clinic communicates to your supervisor when the next exam is due and also informs the supervisor of any recommended work restrictions based on the exam.
Job Certification Examinations
Job certification examinations are for jobs that have specific medical standards, licensing requirements or are mandated by instruction. These include: firefighter, respirator certification, forklift operator, childcare worker, motor vehicle operator, explosives handler and driver, crane operator, etc. The examination frequency is variable and based on the certification. Please bring SECNAV Form 5100/1 to your part 1 exam as noted above in the ‘medical surveillance examinations’ section.
Work-related Injury and Illness Care for Civilian Employees
Civil Service and Non-appropriated Fund employees are eligible for care and follow-up in Occupational Health for their work-related (occupational) injuries. They also have the right to choose their private medical provider for care.
