Family Medicine Medical Home Port (For TRICARE Prime patients enrolled to the hospital)

Family Medicine Medical Home Port offers full-scope primary care services to patients of all ages. This includes preventive health visits, the management of acute and chronic illnesses in the outpatient and inpatient setting, and family centered pregnancy care from the first OB visit through delivery for both the mother and newborn. There is also a broad range of minor surgical procedures and screenings including, but not limited to: vasectomy; colposcopy; and skin biopsies.

Your first appointment with your Primary Care Manager will provide an opportunity for your Primary Care Manager to review your medical history with you and to build a relationship to better serve all of your healthcare needs. 

At each visit please bring:

• A list of all medications or supplements you are taking, both prescribed and over-the-counter.
• A list of all of your questions or concerns. Writing these down before the visit will aid your Primary Care Manager in ensuring that your concerns are addressed and follow-up appointments are arranged when needed.
